We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will be responsible for delivering compassionate patient-centered care while supporting residents daily clinical needs and treatment plans.

This role requires strong clinical judgment attention to detail and the ability to work effectively within an interdisciplinary healthcare team.

• Provide direct nursing care in accordance with care plans and physician orders
• Administer medications and treatments safely and accurately
• Monitor document and report changes in patient condition
• Assist with admissions discharges and care plan updates
• Collaborate with RNs physicians and interdisciplinary care teams
• Maintain accurate and timely clinical documentation
• Follow infection control and safety protocols
• Support patient comfort dignity and overall well-being

• Active Licensed Practical Nurse (LPN) license in the state of practice
• Graduation from an accredited Practical Nursing program
• CPR/BLS certification preferred or required per facility standards
• Strong communication and clinical skills
• Ability to work day shift (07:00 AM 03:30 PM)
• Prior long-term care experience preferred
