Job Overview

We are seeking a reliable Licensed Practical Nurse (LPN) for a travel assignment in a skilled nursing facility. The ideal candidate will provide direct patient care administer medications monitor patient conditions and collaborate with the interdisciplinary care team to ensure high-quality resident outcomes.

This role requires strong attention to detail familiarity with long-term care environments and the ability to work effectively within a structured clinical setting.

Key Responsibilities

Provide direct nursing care to residents according to individualized care plans
Administer medications and treatments as prescribed
Monitor assess and document patient conditions and changes
Assist with admissions discharges and resident assessments
Collaborate with RNs CNAs physicians and care team members
Ensure compliance with infection control and facility safety standards
Maintain accurate and timely clinical documentation
Provide compassionate patient-centered care

Assignment Details

Start Date: 06/08/2026
End Date: 09/05/2026

Shift: Day Shift (Extended)
Shift Time: 07:00 19:00

Schedule: Every Other Weekend Required
Guaranteed Hours: 36 Hours per Week
Contract Length: Approximately 14 Weeks

Requirements

Active Licensed Practical Nurse (LPN) license required
Long-term care or skilled nursing experience preferred
PointClickCare (PCC) experience required
Strong clinical assessment and documentation skills
Ability to work independently and within a team
Must comply with facility dress code (navy scrubs; proper footwear required)
Must bring state or federally issued identification and wear a photo badge at all times
COVID-19 vaccination or approved exemption may be required per facility policy
