With 50 locations and 100 programs in NYCs most under-served communities our 1000 results-oriented professionals make a difference for more than 40000 children and their families each Child Center Residential Treatment Facility (RTF)is a coeducational program in a warm supportive environment that serves young people ages 12-18 who have had multiple psychiatric hospital placements or have been involved with the juvenile justice system. It is the only downstate RTF in the state that is a dual NYS Office of Mental Health/Office of Children and Family Services facility. We never consider it too late to give young people the help they need and we are pleased to partner with these two agencies to ensure the young people in our care have the skills and support they need to achieve long-term success. Reflecting The Child Centers dedication to comprehensive care the RTF is staffed by clinical coordinators psychiatric providers a pediatrician and 24-hour nursing staff 24 hour youth advocates. Supports are individualized and tailored to the needs of the young SummaryThe Transitional Coordinator is responsible for engaging and supporting youth and families in the RTF from entry to through post-discharge from the Residential Treatment Facility (RTF). The TC works hand in hand with the treatment team to bring the family into the center of the treatment and discharge process and secure all entitlements resources and supports for youth and families necessary to ensure positive outcomes post discharge from the FunctionsProvide intensive hands-on work for a caseload of youth and discharge resources/families to ensure a successful transition into the community with a focus on integrating skills learned in the RTF into the discharge settingParticipate in clinical and treatment meetings including but not limited to pre-admissions CTPs and discharge conferencesAttend and escort youth to families homes and schools to check the environment to meet the safety requirements to ensure a smooth transition Attend and escort youth to their community; housing referral interviews medical or school appointments etcAssess family strength & needs assuming primary responsibility for the development and coordination of plans to address impediments to successful reunificationMaintain a current working knowledge of local resources referral procedures and contactsEffectively and efficiently navigate and communicate with systems impacting youth and families including but not limited to OCFS DSS/ACS DOE etcProvide hands-on work to educate and coach youth and families towards a working knowledge of community resources and supports with direct support and linkage to local resources in the childs home community both throughout admission and post dischargeResponsible for the continued scheduling and progressing of family time including transportation needs skill building and behavior management techniques for familiesEnsures all necessary school supports and needs are met maintaining regular contact with the RTF in-house D75 school systemProvide for and maintain tracking of all transitional/wrap-around funds for clients following OMH policies ensuring they are utilized for their intended purposesEnsure compliance with all OMH regulations and guidelinesOther tasks as requiredHoursSunday Thursday 10:00 AM 6:00 PM Flex hours may be needed depending on the days workWork at least 1 weekend day out of the weekQualificationsMasters Degrees in human services field with at least 2 years of experience providing direct services to or linking children with SED to servicesMust be over 21 years of ageLMSW or LMHC Certification requiredAbility to write and communicate effectivelyKnowledge of Microsoft Office Suite (Outlook Word Excel Power Point)Exercises independent initiativeStrong attention to detailAbility to remain calm under pressure and stressful situationsDemonstrates flexibility and patience to meet demands of the workAbility to influence and motivate staffStrong analytical skillsStrong relationship builder (internally and externally)Strong decision-making skills and resourcefulness in the performance of job dutiesPositive attitude concern for people and community with demonstrated presence and self- confidenceAlignment with Diversity Equity and Inclusion principlesIntegrating the mission of the Child Center of NY into communicationWhat We OfferGenerous Paid Time OffMedical/Dental/Vision Insurance401k Pension and Employer ContributionFlexible Spending and Commuter Benefits AccountsEmployer paid short-term & long-term disability life and AD&D insuranceCommuter BenefitsEmployee Assistance ProgramThe Child Center of NY is an Equal Opportunity Employer.
