Transitional Care Nurse Coordinator Opportunity:

Full-time direct hire position
Schedule: Monday-Friday 9a-5p
Transitional Care Nurse Coordinator will lead and implement the swing bed program acquire hospital referrals and assess them for their quality and cost efficiency and oversee the admission and discharge of referred patients
Given an expense account and travel budget to travel to nearby areas in order to expand relationships and referral opportunities

Transitional Care Nurse Coordinator Requirements:

Must hold a Bachelors degree in Nursing
Active unencumbered RN License
Reliable transportation a valid drivers license and vehicle insurance are required
2 years of experience as a Transitional Care Nurse Coordinator case manager or patient navigator
Experience in rural communities and healthcare sales/marketing is preferred
