Responsibilities

• Provide personal care and assistance with activities of daily living (bathing dressing feeding mobility).
• Offer companionship and supervision support in the clients home or facility.
• Monitor client health status and report any changes immediately to the coordinator.
• Document activities and observations in Well-Sky.
• Maintain active and timely communication with the scheduling team.

Mandatory Requirements

Candidates who do not meet these requirements will be automatically disqualified.

• Valid drivers license required; no license no further process.
• Reliable personal transportation to travel between clients in the Mansfield area.
• Eligibility to work in the U.S. documentation required to complete the I-9.
• Schedule availability for morning afternoon night shifts and weekends.
• Social Security Number (SSN) required for background check and payroll.
• Active cell phone for communication with the team and clients.

Required Documents for the Hiring Process

All candidates must submit the following documents through the application portal:

Identity & Work Eligibility (I-9)

• Valid government-issued ID.
• Social Security card.
• U.S. passport.
• Drivers license.
• Auto/car insurance.

Tax Documents

• W-4 Form (federal tax withholding) completed during onboarding.

Background Check

• Signed background check authorization mandatory before starting any shift.

Onboarding Documents

• Signed company policies and employee handbook.
• Direct deposit banking information (optional but recommended).

Certifications

• CNA HHA or CPR/First Aid certification (if applicable).
• At least 1 professional reference from a previous caregiving role.
