Job Summary
We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU and Operating Room. The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily responsibilities include patient assessments medication administration treatment planning and collaboration with the interdisciplinary team to ensure optimal patient outcomes. Strong communication and teamwork skills are essential along with at least two years of relevant experience.
Responsibilities:- Perform comprehensive patient assessments and monitor patient conditions.
- Administer medications and treatments as prescribed and document accordingly.
- Develop and implement individualized care and treatment plans.
- Collaborate with interdisciplinary teams including physicians therapists and social workers.
- Provide case management and coordinate hospice care services.
- Maintain accurate and timely patient records using electronic medical systems.
- Educate patients and families on health conditions and care procedures.
- Ensure compliance with hospital policies procedures and regulatory requirements.
- Participate in quality improvement initiatives and staff meetings.
- Active Registered Nurse license in the state of practice.
- Bachelors degree in Nursing or related field.
- Minimum of two years of experience in hospital home health or hospice care.
- Proficiency in clinical assessment medication administration and treatment planning.
- Strong interdisciplinary team collaboration skills.
- Excellent communication and interpersonal abilities.
- Valid drivers license car insurance and automobile for field work.
- Experience in specialized nursing areas such as ICU ER Psychiatric Labor & Delivery PACU or Operating Room.
- Familiarity with Epic electronic medical record system.
- Additional certifications related to nursing specialties or hospice care.
- Position requires working in a fast-paced acute care environment.
- Collaboration with various hospital units including critical care and maternal child.
- Scrub color requirements vary by unit; scrubs provided for specific departments.
- On-call is not required for this role.
- Weekend shifts are required with at least one regular weekend shift per week.
- Expected patient care ratio is 1:1 in some cases.
- Work schedule is Tuesday through Saturday.
- First shift (day shift) hours.
- No block scheduling; shifts assigned per unit needs.
- 48-hour workweeks are not approved for this position.
