Job Summary

  • Pain & Symptom Management: Assess patients and administer medications/treatments to ensure comfort and alleviate suffering.
  • Care Planning: Develop and update customized patient care plans in collaboration with physicians social workers and other team members.
  • Family Support: Educate family caregivers on disease progression what to expect and medication management.
  • Crisis Intervention: Respond quickly to sudden changes in a patients condition to manage pain and provide crisis support.
  • Documentation: Keep accurate up-to-date patient medical records and facilitate communication between the care team.
  • Emotional & Spiritual Care: Offer compassionate companionship grief counseling and post-mortem support to grieving relatives.

    Requirement description :
    • 2 years Hospice experience - REQUIRED

    CERTIFICATION REQUIREMENTS :
    • BLS (AHA)

    STATE LICENSE REQUIREMENTS :
    • Michigan

    ADDITIONAL LICENSE REQUIREMENTS : Active at time of Submission
    Weekend Requirements : One week total of evening and weekend call per month. Weeknights 1700-0830 Weekend 24 hr./day.
    On Call Requirements : On-call 1 full week every month
