Job Summary

Medication Order Verification: Review and interpret electronic physician orders to confirm accurate drug choices safe dosages and proper administration routes.

Clinical Monitoring & Interventions: Evaluate patient profiles for allergies drug-drug interactions and lab results. They frequently perform pharmacokinetic dosing for high-risk medications.

Compounding & Dispensing: Oversee or personally prepare specialized medications including large-volume parenterals intravenous (IV) piggybacks total parenteral nutrition (TPN) and chemotherapy infusions under strict sterile guidelines (such as USP < 797>/< 800>).

Interdisciplinary Collaboration: Participate in medical rounds with physicians and nurses to recommend evidence-based therapies and assist in medical emergencies.

Transitions of Care: Conduct medication reconciliation during a patients admission and provide comprehensive drug counseling to patients upon hospital discharge.

Staff Supervision: Manage review and delegate tasks to pharmacy technicians interns and students to maintain accurate inventory and cart distributions

Requirement description :

2 years inpatient pharmacist experience - REQUIRED
Able to work in the Anticoagulation Clinic (4-6 pts a day) and perform Oncology compounding (pharmacist do all the IV compounding) - REQUIRED
