- 1 Supervisory Signed Reference from the past year of employment REQUIRED
- Active American Heart Association BLS - REQUIREDDOCUMENT
- National Certification (ASCP preferred) REQUIREDDOCUMENT
- Bachelors Degree REQUIREDDOCUMENT
- All licenses/certs must be current and in hand at time of submission
- Permanent address must be included in submission file REQUIRED
- RTO must be listed in submission
