Were looking for compassionate customer-focused professionals to join our team as a Tailored Care Manager Extender! In this role youll be the friendly voice on the other end of the phoneconnecting members to the care services and support they need to live healthier lives.

Make outbound and receive inbound calls with members and healthcare partners.

Build positive relationships through compassionate professional communication.

Assist Tailored Care Managers by completing outreach scheduling appointments coordinating services and documenting member interactions.

Help connect members with healthcare providers community resources and essential support services.

Maintain accurate documentation while meeting quality and productivity goals.

Excellent communication and active listening skills
A passion for providing exceptional customer service
Comfortable working in a high-volume call center environment
Strong computer and multitasking abilities
Experience in healthcare customer service call centers or care coordination is a plus
