Targeted Case Manager

Description:The Targeted Case Manager provides Rule 79 Case Management services to eligible persons primarily referred directly from Hennepin County or self-referral who have been diagnosed with a serious and persistent mental illness. TCM services assist with access to medical social and other services. Targeted Case Managers ensure continuity of care coordination and monitoring of services discharge planning housing support and intensive goal planning through the development of the Individual Community Support Plan. The Targeted Case Manager would utilize a person-centered approach to work in partnership with persons served with the goal of improved functioning through acquisition of necessary supports skill development and continuity of services. *Here at SpringPath we provide services to people-served who are experiencing significant mental health/psychiatric symptoms who are demonstrating significant functional impairments are often experiencing complex challenges with substance use poverty being unhoused and often intersect with the criminal justice system. This Generalist Case Manager/PRP is assigned to persons served individualized treatment teams and work along-side other specialists in carrying out rehabilitative interventions. As part of the person(s) served individualized treatment team the Case Manager/PRP is responsible for conducting assessments that monitor the person(s) served s goals and functioning on an ongoing basis. The Case Manager/PRP typically sees approximately 16 to 20 persons served within 1 working week and is expected to communicate interactions with the person served at daily team meetings and documenting these interventions in the Electronic Health Job Responsibilities:Along with using a person-centered approach and building meaningful relationships our Targeted Case Managers will have the following job duties:*Conducting a variety of clinical assessments to determine needed areas of the ICSP (Individual Community Support Plan)*Development of the Individual Community Support Plan (Treatment/Rehabilitation Plan)*Referral and linkage to community resources*Monitoring and coordinating of resources supports and services*Civil Commitment responsibilities**building meaningful relationship; knowledge and skill with benefit acquisition and management; resource and referral; knowledge of Rule 79 clinical a bachelors degree in one of the behavioral sciences or related fields from an accredited college or university and/or has at least 2000 hours of supervised experience in the delivery of services to persons with serious and persistent mental illness; or holds a graduate degree in one of the behavioral sciences or related fields. Individuals with 2000 hours of experience in the delivery of mental health services to adults All staff are supported in gaining certification as a Certified Psychiatric Rehabilitation Practitioner. Must possess sufficient eye/hand coordination to operate office adaptive and household equipmentMust be able to bend stoop push pull reach sit and walk for periods of time. Must be able to utilize proper body mechanics while lifting up to 75 pounds. Home-like and office settings with varying degrees of background noise. Light and ventilation as found in typical home or office setting. Smoking areas are designated at work may exist if the employee fails to use proper body mechanics when lifting or transporting an individual or item if vehicles are not operated safely or if proper techniques are not used when intervening with possibility of blood borne pathogens exists if universal precautions are not : $63000-$65000
