Job Description:

SCOPE: The Survivorship Coordinator will lead the development implementation and ongoing management of a comprehensive cancer survivorship program beginning within our breast clinic and eventually expanding across the Mary Bird Perkins Cancer Center network. This individual will serve as a key facilitator between clinical teams patients and community resourcesensuring that survivorship care plans are meaningful evidence-based and scalable across our enterprise.

2. Facilitation and Coordination

3. Research and Resource Development

4. Institutional Engagement and Leadership

5. Education and outreach

6. Other duties as assigned


QUALIFICATIONS:

Degree in Nursing Social Work Public Health or a related field required

EXPERIENCE:

  • Minimum 3 years of experience in oncology or chronic care coordination; survivorship experience strongly preferred.
  • Strong project management research and organizational skills. Analytical skills to gather and summarize information for planning & development
  • Excellent communication and interpersonal skills with the ability to work across departments and levels.
  • Proficient in EMR systems and data reporting.
