Support Worker

****Average hourly is $18.32 (including wage incentives bonuses overtime shift differential etc.)****

Position Summary

Provides direct support to families in the Family Preservation Program to promote child safety stability and well-being. Works collaboratively with case managers therapists and community partners to ensure families receive necessary services and meet case plan goals.

Key Responsibilities

• Provide daily direct and indirect support to families in home school and community settings
• Coordinate community resources (e.g. healthcare childcare housing employment benefits)
• Collaborate with case managers and therapists to implement case plans
• Offer emotional support coaching crisis intervention and parenting guidance
• Assist families with accessing services such as SNAP Medicaid SSI and employment support
• Maintain accurate and timely documentation and prepare records for audits
• Identify and address barriers to family engagement and service success
• Transport clients as required and attend court proceedings when applicable
• Support ongoing communication between families and service providers
• Promote teamwork cultural sensitivity and agency values

Qualifications

• High School diploma or GED required
• Valid drivers license and ability to transport clients
