The Loudoun County Department of Mental Health Substance Abuse and Developmental Services is seeking a talented and motivated individual to serve as a Support Coordinator in the Support Coordination this role you will work with a trusted and reputable team that has a history of success connecting individuals and their families with person-centered recovery-oriented services and supports.

The Department of Mental Health Substance Abuse and Developmental Services seeks a Support Coordinator with the Developmental Services Division. This position is responsible for assessing coordinating monitoring and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers CCC Plus Waiver and Medicaid is preferred. Additionally knowledge of community-based resources including integrated housing options and employment first principles is desired.

Duties may include:

• Assessing eligibility and the needs of individuals.
• Assessing possible individual risks.
• Interviewing individuals and their families.
• Facilitating team meetings.
• Coordinating service delivery among providers and developing and documenting person-centered plans using a strengths-based approach.
• Assuring quality service provisions.
• Maintaining compliance with all State of Virginia Licensure standards Medicaid standards and crisis intervention and prevention.
  

• Demonstrate effective oral and written communication skills.
• Have strong organizational and time management skills.
• Haveproficiency in utilizing electronic health records and other software programs for documenting service delivery.

Bachelors degreein Human Services Child Development Early Childhood Special Education Social Work Counseling Psychology or related field and three (3) years of related experience in assigned program area or equivalent combination of education and experience.

Preferred Qualifications:

• Masters degree in Human Services Child Development Early Childhood Special Education Social Work Counseling Psychology or related field.
• Three (3) years of related experience in assigned program area as a casemanager service coordinator or multi-disciplinary team facilitator with children youth adults and families.

Employment is contingent upon successful completion of background checks including fingerprinting NCIC credit and DMV checks. The successful candidate must have a valid drivers license in Virginia or regional state of residence.
