What Youll Do
Build strong relationships with individuals and families while developing person-centered Individual Support Plans (ISP/PCPs) that reflect each persons goals strengths and preferences.
Coordinate services and community resources that promote independence inclusion and an enhanced quality of life.
Collaborate with individuals families Conflict-Free Case Management (CFCM) providers and interdisciplinary teams to ensure supports are coordinated responsive and effective.
Monitor services advocate for individuals rights and help navigate changing needs and opportunities.
Educate individuals and families about available resources and supports while ensuring services remain person-centered.
Maintain accurate documentation and compliance with agency and Rhode Island regulations.
Participate in ongoing professional development and contribute to a collaborative supportive team culture.
What Were Looking For
Bachelors degree in Human Services Psychology Special Education or a related field preferred.
35 years of experience in human services preferably supporting individuals with intellectual and developmental disabilities.
Knowledge of person-centered planning support coordination and Rhode Island service systems preferred.
Strong communication organization advocacy and relationship-building skills.
Proficiency with Microsoft Office Google Workspace and electronic documentation systems.
A genuine commitment to person-centered services and the belief that every individual has unique strengths valuable contributions to make and the ability to achieve meaningful life outcomes.
