Job Summary: Provides counseling services including identifying screening and assessing clients to determine the need for substance use treatment. Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records
Duties and Responsibilities:
- Conduct intake interviews biopsychosocials mental health screenings
- Formulate treatment plans and treatment plan reviews act as participants case manager to coordinate care within Crossroads and will external providers
- Perform individual counseling duties which include holding weekly individual sessions with clients providing referrals to community-based resources (healthcare social services and administrative e.g. help obtaining insurance) and developing client schedules
- Initiate and maintain client records in accordance with policies and procedures Ensure accurate and timely data entry of all client information into the agencys database; utilize electronic health record system
- Facilitate assigned treatment groups
- Perform drug testing as needed
- Prepare monthly updates for court or other supervising agencies
- Participate in treatment team case reviews.
- Consult with other professionals in support of clients treatment
- Complete discharge documentation as needed
