  • CREC is expanding its team ofLicensed Practical Nurses in our Strive Program in Clinton.

    The STRIVE Program partners with local school districts families young adults state agencies and community resources to provide comprehensive transition planning and services. STRIVE prepares young adults for post-secondary life by assisting them in realizing their potential addressing their needs and meeting goals related to daily living career planning and opportunities for adult learning.

    We are currently seeking a full time licensed practical nurse to provide appropriate care for ill medically fragile and/or injured students; providing health information and serving as a resource to parents teachers staff and administrators; and identifying health problems for referral for proper treatment while working within their scope of practice and under the direction of a registered nurse within the building or the Assistant Director of Health Service in a nurses absencefacilitate and strengthen the educational process by improving and protecting the health status of children and by identifying and assisting in the removal or modification of health-related barriers to the learning process for each child.     JOB DUTIES INCLUDE (but not limited to)

    • Administers mandated screenings (e.g. vision postural hearing etc.)
    • Assesses situations involving students safety abuse (physical sexual drug etc.) and other health related issues.
    • Counsels students and families on health conditions and lifestyle issues.
    • Implements the health care plans developed by the registered nursefor students with IEP and emergency care plans.
    • Maintains contact with parents/guardians for the purpose of advising them of changes in student health.
    • Maintains students confidential files and recordsrequired by legal requirements and professional standards.
    • Participates in a variety of meetings workshops and seminars Individual Educational Plan meetings and interdisciplinary teams.
    • Provides care to students with chronic illnesses and extremely high-risk students.
    • Provides training on a variety of health related subjects for the purpose of promoting a healthy lifestyle and/or acting as a resource to students teachers and other school personnel.
    • Refers students requiring additional medical attention for the purpose of providing required follow-up treatment and services.
    • Reports health and safety issues to assigned administrator and appropriate agencies.
    • Researches a variety of topics relating to health and medical care.
    • Responds to emergency medical situations.
    • Adheres to all applicable professional safety standards and practices within their scope as an LPN including requirements set forth in federal and state law and Occupational Safety and Health regulations.
    • Reports to a registered nurse to receive appropriate direction of care for a student.



    JOB SPECIFICATIONS AND CORE COMPETENCIES

    Education: Community College and/or Vocational School degree with study in job related area.
    Licensure: Current LPN Connecticut license.
    Experience:Job related experience with increasing levels of responsibility is required. Emergency Department or Critical Care experience preferred. One year recent acute care experience preferred. Previous school nursing desirable.

    Other Job Requirements:
    Ability to communicate appropriately with students parents school staff and CREC staff is required. Demonstration of interpersonal skills necessary for communication with media visitors and community partnership representatives required.
    Assigned hours and location within your shift starting time or days of work are subject to change based on departmental and/or organizational needs.
