We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN this role you will evaluate and treat residents with communication cognitive and swallowing disorders while collaborating with physicians nursing staff families and other healthcare professionals to improve resident outcomes and quality of life.
- Evaluate residents and develop individualized treatment plans
- Provide speech language cognitive and swallowing therapy services in accordance with physician orders
- Monitor resident progress and modify treatment plans as appropriate
- Communicate with physicians nursing staff residents families and interdisciplinary team members regarding treatment goals and outcomes
- Complete accurate and timely documentation in compliance with facility regulatory and payer requirements
- Participate in care planning meetings and discharge planning activities
- Educate residents families and caregivers on treatment plans and maintenance programs
- Maintain compliance with all state practice acts facility policies and regulatory requirements
- Promote resident safety independence and quality of life
- Masters or Doctoral degree in Speech-Language Pathology required
- Must meet one of the following requirements:
- Licensed as a Speech-Language Pathologist in the state in which services are provided
- Experience in skilled nursing rehabilitation or long-term care preferred
- Experience with PointClickCare (PCC) and Casamba preferred
- Strong clinical documentation and communication skills
- Current Speech-Language Pathologist license in good standing
- Must maintain all required continuing education requirements
- Must remain in good standing with the applicable State Board
- Passion for helping residents improve communication cognition and swallowing function
- Strong clinical judgment and problem-solving skills
