Position Summary

Concept Rehab is seeking a PRN Speech-Language Pathologist (SLP) to join our rehabilitation team at a skilled nursing facility in Johnston Iowa. This role offers exceptional flexibility for clinicians seeking supplemental income schedule control or work-life balance while delivering high-quality speech therapy services.


The Speech-Language Pathologist evaluates and treats residents with communication cognitive swallowing and speech disorders. The SLP works collaboratively with physicians nursing staff families and the interdisciplinary care team to maximize patient outcomes and quality of life.


Requirements

Key Responsibilities

  • Evaluate residents and establish individualized treatment plans.
  • Provide speech language cognitive communication and swallowing therapy services.
  • Complete accurate and timely documentation progress notes and billing.
  • Collaborate with physicians nursing staff families and interdisciplinary team members.
  • Participate in care planning and discharge planning meetings.
  • Educate residents caregivers and facility staff on treatment strategies and maintenance programs.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Maintain compliance with all federal state facility and company guidelines.
  • Support a positive patient-centered rehabilitation environment.
Qualifications
  • Masters Degree in Speech-Language Pathology.
  • Current ASHA Certificate of Clinical Competence (CCC-SLP) preferred.
  • Current Iowa Speech-Language Pathology license or eligibility to obtain licensure.
  • Experience in skilled nursing long-term care rehabilitation or geriatric settings preferred.
  • Strong communication documentation and clinical decision-making skills.
  • Passion for providing exceptional patient care.
