Job Responsibilities:
Evaluate diagnose and treat speech language communication cognitive and swallowing disorders in a Skilled Nursing Facility setting
Develop and implement individualized treatment plans based on patient needs and clinical findings
Provide therapy focused on speech production cognitive communication memory language and dysphagia management
Collaborate with physicians nurses therapists dietitians and interdisciplinary teams to support patient recovery
Educate patients and caregivers on communication strategies swallowing precautions and home exercise programs
Monitor patient progress and modify treatment plans as needed
Maintain accurate and timely documentation in compliance with facility and regulatory standards
Requirements
Active state license as a Speech-Language Pathologist in California
ASHA CCC-SLP certification preferred or required depending on facility policies
Current BLS/CPR certification preferred
Minimum of 1 year of recent SLP experience preferred ideally in a SNF rehabilitation or geriatric setting
Strong communication clinical reasoning and patient care skills
