Seneca Health Care Center is seeking a compassionate and dedicated Social Worker to support residents and families through every stage of their care this role youll assess psychosocial needs coordinate services facilitate care planning and help residents achieve the highest possible quality of life in a supportive resident-centered environment.

RESPONSIBILITIES:
Complete psychosocial assessments and social histories upon admission
Assist residents and families with advance directives care planning and transition support
Facilitate care plan meetings with residents families and the interdisciplinary team
Provide counseling support and interventions related to emotional social and behavioral needs
Coordinate discharge planning and referrals to community resources and home care services
Complete MDS assessments and documentation as assigned
Document resident and family interactions in the electronic medical record
Serve as a liaison between residents families staff and outside agencies
Participate in QAPI initiatives and contribute to quality improvement efforts
Collaborate with all disciplines to promote resident well-being and positive outcomes
REQUIREMENTS:
Bachelors Degree in Social Work Human Services Psychology or related field required
LMSW BSW or MSW preferred
Minimum of one year of experience (paid or volunteer) working with a geriatric population in a healthcare setting
Experience with MDS discharge planning and electronic medical records preferred
Strong communication organizational and problem-solving skills
Ability to build positive relationships with residents families and interdisciplinary team members
