Job Type: PRN Nonexempt

Schedule: Days 8:00am - 4:30pm as needed to provide coverage during PTO/time off and based on patient census. Must be available to pick up weekdays and weekends.

*Medical Social Work experience in an acute care or healthcare setting is required.

How youll contribute

The Social Worker is a key member of the Case Management team and is responsible for assessing the social emotional financial and environmental needs of patients and their families to support safe effective care transitions. This role collaborates closely with physicians nurses case managers and community partners to develop and coordinate individualized discharge plans connect patients with appropriate resources and help address barriers to recovery and ongoing care. The Social Worker provides counseling crisis intervention patient advocacy and education while ensuring compliance with regulatory requirements and organizational standards. This position requires strong critical thinking sound judgment and the ability to work independently in a fast-paced acute care hospital environment.

Applicants should have

• Bachelors degree required Masters preferred

• Board Certified Social Worker or related field (i.e. counseling) in the state of North Carolina within 6 months of hire

• Handle With Care within 30 days of hire
