Position: Social Worker

Job Type: Full Time

NHC Place at the Trace is looking for a Social Worker/Discharge Planner to join our team! The qualified applicant for this position should hold a Bachelors degree or Masters in social from an accredited program or a related degree.

Experience preferred with a case management background in a SNF / ICF setting knowledge of MDS and clinical assessments and documentation care planning discharge planning working with support agencies such as hospice home health and mental health services etc.

Responsibilities include but are not limited to:

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
• Working with individual patients and their representatives toward transition back to the community or adjustment to center life and helping navigate crisis of illness and disability
• Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
• Coordinating patients plan of care with interdisciplinary team members
• Reporting to the Director of Social Work current case management status and needs
• Maintaining confidentiality of professional information
