• Split coverage between Sioux Falls and Moccasin Creek Dialysis Clinics
• Monday - Friday schedule
• Full time status - 30 hours a week
• 60 patient case load

• Life on dialysis can be a difficult transition for many patients-and youll be there to support and advocate for them.
• Help patients understand their rights and responsibilities and guide them in managing the physical mental emotional and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.
• Be a part of a Team that appreciates supports and relies on each other in a positive environment.

Requirements:

• Licensed or registered in the state of practice as required by state regulations -- LMSW or LCSW
• Meets all state required regulations to practice in the dialysis setting.
• Masters degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word Excel PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

• Compensation for the role will depend on a number of factors including a candidates qualifications skills competencies and experience and may fall outside of the range shown.
