Social Worker LCSW

CHI St. Alexius Health Dickinson part of CommonSpirit Heath a national nonprofit health system based in Englewood Colorado. The faith-based system operates in 18 states and includes 103 hospitals. Additional services offered within the system are: long-term care assisted and residential living communities community health services organizations home health agencies and numerous outpatient facilities.

As our Social Worker you will be a compassionate advocate providing vital support and guidance to individuals and families facing complex challenges within the healthcare setting. Your expertise in navigating social emotional and practical issues will be crucial to promoting overall well-being and successful patient outcomes.

Every day you will conduct psychosocial assessments develop individualized care plans provide counseling and crisis intervention and adeptly connect clients to essential community resources. Youll be a strong advocate for clients needs collaborating seamlessly with multidisciplinary teams to ensure holistic support and promote their well-being throughout their healthcare journey.

Responsibilities:

• Conducting comprehensive psychosocial assessments to identify patient and family needs strengths and challenges.
• Developing and implementing individualized care plans that integrate medical social and emotional needs working collaboratively with patients families and the healthcare team.
• Providing supportive counseling crisis intervention and emotional support to patients and families coping with illness trauma loss and other life changes.
• Facilitating access to appropriate community resources services and support networks including discharge planning long-term care financial assistance and mental health services.
• Advocating on behalf of patients and families to address barriers to care ensure access to necessary services and protect their rights and dignity.
• Documenting all social work interventions assessments and care plans accurately and timely in the electronic health record adhering to professional standards and hospital policies.

• Bachelors Degree in Social Work
• Licensed Clinical Social Worker: ND (LCSW:ND)

Preferred:

• Masters Degree in Social Work
