Join One of Northeast Floridas Premier Senior Care Communities
River Garden Hebrew Home an award-winning not-for-profit five-star nursing and rehabilitation facility located in Jacksonvilles beautiful Mandarin area is seeking an experienced and dynamic Social Worker to join our Social Services Department.
As a Social Worker in our facility youll play a key role in helping residents and their families navigate care transitions emotional challenges and support services. This is a unique opportunity to make a meaningful difference in the lives of both short-term rehab patients and those requiring long-term support.
What Youll Do:
- Provide emotional support and counseling to residents and families
- Coordinate care plans in collaboration with our interdisciplinary team
- Advocate for resident needs and preferences
- Ensure documentation complies with regulatory standards
What Were Looking For:
- A caring empathetic personality with a genuine desire to help others
- Strong communication and interpersonal skills
- Ability to handle sensitive situations with professionalism and compassion
- Bachelors or Masters degree in Social Work (BSW or MSW)
- 2-3 years experience in a nursing home or healthcare setting preferred
