Rate of Pay: $28.95 to $33.00/hour based on degree licensure and experience

Job Description

As a Social Worker youll be at the forefront of building meaningful relationships with our residents their families and designated representatives. Youll be their go-to person during important transitions between different levels of residency or other life changes offering guidance support and a friendly ear to listen. But thats not allyour role as a valued member of our interdisciplinary team will also involve contributing to Person-Centered Care Planning and with our Director of Social Work ensure we meet all regulatory requirements for our 48-bed skilled nursing department within our vibrant life plan community. Youll assist with quality assurance initiatives facilitate various resident support groups and staff education as needed and act as a liaison to the Long-Term Care Ombudsman program.

Qualifications

• Masters or Bachelors degree in social work with 2 years of experience
• Experience in long-term care health care end-of-life care or gerontology
• Preferred but not required LMSW
• Excellent interpersonal and time management skills
• Ability to collaborate effectively within an interdisciplinary team
• Proficiency in computer programs such as Word Excel Outlook etc.
• Familiarity with Electronic Medical Record Software
