What Youll Do:
In this role youll play an important part in supporting the emotional and social well-being of skilled nursing residents by ensuring their individual needs are understood and addressed. Youll build meaningful relationships collaborate with healthcare professionals and help deliver person-centered care that promotes comfort dignity and quality of life.
Key Responsibilities
- Conduct psychosocial assessments and develop individualized care plans that support residents emotional social and overall well-being.
- Partner with residents families and interdisciplinary care teams to identify needs establish goals and coordinate services.
- Provide guidance counseling advocacy and support to residents and their families throughout their healthcare journey.
- Connect residents with community resources financial assistance programs and specialized services to enhance quality of life.
- Coordinate discharge planning and care transitions to ensure a smooth and successful move to the next level of care.
- Advocate for residents rights preferences and psychosocial needs while promoting dignity and person-centered care.
- Maintain accurate documentation and ensure compliance with federal state and local regulations as well as organizational policies.
- Participate in quality improvement initiatives team meetings training programs and collaborative efforts that support exceptional resident care.

Skills Knowledge and Expertise
Required Qualifications
- Bachelors degree from an accredited School of Social Work OR a bachelors degree in a related human services field such as Social Work Sociology Psychology Rehabilitation Counseling Special Education or a closely related discipline.
Preferred Qualifications
- One (1) year of supervised social work experience in a healthcare setting working directly with individuals.
- Experience working with geriatric skilled nursing long-term care or rehabilitation populations.
- Strong communication advocacy care coordination and problem-solving skills.
