Pay: $32.00-$43.00/hour

Social Worker Opportunity:

Full-time permanent position
Monday-Friday working 8:00AM-5:00PM
The Social Worker will assess patients psychosocial needs and provide counseling emotional support and individualized care planning
Connect patients and families with community resources while advocating for their social financial and support needs
The Social Worker will collaborate with the interdisciplinary care team to coordinate care maintain documentation and support quality patient outcomes

Social Worker Requirements

Masters Degree in Social Work Required
LCSWs are preferred but LMSWs CSWAs and ACSWs are welcome to apply
