Social Worker- Blank Family Services
Join our Blank Family Services team as a Social Worker where youll play an important role in supporting patients and families through some of their most meaningful and challenging moments. As part of an interdisciplinary care team youll use your professional and clinical expertise to assess psychosocial needs reduce barriers to care connect individuals with community resources and advocate for safe supportive transitions across the hospital setting.

This role offers the opportunity to work in a variety of hospital-based care areas while providing direct support emotional guidance transition planning and resource coordination that can make a lasting difference for patients and their families.
What Youll Do:
- Assess patient and family psychosocial needs and identify barriers that may impact health healing and discharge planning.
- Connect patients and families with community resources support services and programs that promote access to care and long-term stability.
- Collaborate with physicians nurses care teams and community partners to support coordinated patient-centered care.
- Provide emotional support advocacy and guidance to patients and families navigating complex health care situations.
- Assist with transition planning and facilitation to support safe timely and effective movement across levels of care.
Education: MSW degree Required in Social Work from an accredited school.
Experience: Prefer one year clinical experience in health care and with pediatric population. Experience conducting mental health assessments.
License(s)/Certification(s): Valid LBSW LMSW LISW or similar licensure through State of Iowa active at time of hire. Successfully complete Person Centered Care course within 1 year of hire. Valid mandatory Reporter course completion by state(s) requirement.
