Social Services Designee

Citizens Memorial Hospital (CMH) is hiring aSocial Services Designee to join ourSocial Services Department inStockton Missouri someone who leads with skill compassion and a drive to positivelyimpactevery patient and family we serve.

Position Details

• Status:Part Time

• Work Shift:Days

• Hours:8:4:00p.m 2-3 days/week

What Your Day Looks Like:Lake Stockton Healthcare Facility (LSHCF) is a 90-bed long-term skilled nursing facility. A 30-bed memory care unit meets the specific needs of individuals with Alzheimers disease and other forms of dementia.

As aSocial Services Designee you will be:

• Assistingwith admission/discharge planning

• Coordinating the transportation of residents to appointments

• Delivering residentmailthe same day as received

• Assessing each residents psychosocial status

• Serving as a liaison between the families of residents and the health care team in time of crises

• Making arrangementsfor obtaining needed adaptive equipment clothing and personal items

• Assistingwith family/resident concerns and/or needs

• Utilizing the communities resources toassistin the discharge of short stay residents

Qualifications:

Education: High school diplomaMHCA Designee Trainingrequired willassistin obtaining

Experience: 1 year of long-term care/healthcare preferred

Skills: Collaborative detail-orientated problem-solving

Licensure/Certification: Family and Friends CPR within 6 months of employment

Physical Requirements: Ability to stand for 10% of the day and sit for the remaining 90%
