Seeking an experienced Social Services Designee whose passion is supporting the holistic emotional and psychosocial well-being of each resident at a 99-bed Skilled Nursing Facility located in the Beverly Hills area.
Key duties include:
- Conducting psychosocial assessments
- Navigating the complex Medicare/Medicaid resources alongside families
- Participating the team care planning process
- Coordinating safe discharge plans
- Resident and family counseling & support
- Regulatory compliance and documentation
- Advocacy and grievance interventions
