POSITION PURPOSE:
Social Workerassistsresidents and their families with emotional and social needs and ensures that needs are met andmaintainedon an individual basis. The Social Worker integrates social services with other elements of the care plan tomaintainthe highestfeasiblephysical mental psychosocialwell-being of each resident.
ESSENTIAL FUNCTIONS OF POSITION:
Confers with resident and family membersto develop a comprehensive social history and psychosocial assessmentincludingthe residents concerns andculturalreligiousand ethnic background preferences.
Orientsresidents and their families to the facility its services limitations and resident rightsto ensure a smooth transition during admission to the facility.
Completes sub-sections of the MDS assessmentand accurately enters into the computer system conferring with a nurse as needed.
Completes thePre-admissionscreening and resident review (PASRR) for specialized services for Medicaid todetermineeligibility.
Meets all submission timelines of reports/records/documentation as required by state and federal rules regulations and laws for healthcare facilities.
Assistswith the developmentimplementationand evaluation of the care plans for each residentin accordance withother health care providers and physicians.
Empowers andencourageresidents to be proactive in their care plans and decisionseffectingthem and being receptive and sensitive to patient requests.
Providessupport for residents and family membersexhibitingemotional distress.
Participates in quarterly care plan conferences for each resident including family physician and care plan team.
ActivelyparticipatesinQAPI(Quality Assurance Process Improvement) program taking a proactive approach to quality care and services on a continuous basis with engagement of residents familymembersand staff.
Demonstrateshigh standardsof excellence for meetingresidentneeds. Communicates effectivelyestablishesrapportandmaintainsrelationships with residents.
Meetsrequirementof regular attendance to perform job duties.
Hasfun increatinga positive atmosphere forresident.
Followsinstructions and completesdaily assignments as assigned andisproactive to change.
Possessesunderstanding and compassion for the elderly population.
Performs other duties as assigned.

Qualifications
EDUCATION/REQUIREMENTS
Bachelors Degree in Social Work Human Services or related field (required).
One year of supervised social work experience in a health care setting working directly with individuals (preferred)
License/Certifications
Iowa Board of Social Work licensed (preferred).
CPR Certification (preferred).
