Summary
The purpose of SOAR is to improve access to SSI/SSDI for homeless individualsespecially those affected by substance abuse and mental health conditions who often experience difficulty in accessing such benefits.
What You Will Do to Help End Homelessness
Identify clients that are eligible for SOAR program.
Provide intensive assistance through the application process.
Collaborate with partner agencies including the Social Security Administration.
Track and enter data as it relates to SSI/SSDI applications.
Write medical summary reports to support SSI/SSDI applications.
Communicate with clients SSA Medical Records Departments and other agencies to complete the SSI/SSDI application process.
Participate in weekly/monthly SOAR Coordination Meetings.
Liaise with SOAR technical assistance (TA) specialist.
Assist Housing Program Manager with SOAR presentations and trainings.
Youre An Ideal Fit If You Have
Bachelors degree in human services field or Public Administration or related discipline or equivalent in experience
Familiarity with the Social Security Administration and processes for applying for state and local benefits and entitlement programs
Experience working with DDS SSA Medical Records Departments and other agencies to complete the SSI/SSDI application process
Strive for innovation and think outside the box.
Experience working in areas of crisis with vulnerable populations
Multi-lingual (Spanish is a plus)
Personal Experience with Homelessness
Have experience working with populations experiencing homelessness
Non-Profit Background
Superior written and verbal communication skills (Spanish a plus).
Demonstrated ability to communicate effectively and professionally with staff clients and other external contacts.
Ability to handle sensitive material maintaining the highest level of confidentiality and displaying sound judgment.
Valid Drivers License/Clean 3-year MVR
Mobility sufficient to conduct regular duties within a normal office environment.
Ability to lift up to 20 pounds in order to safely manage office supplies and equipment
