Job Summary

Service Access Coordinator

Schedule: Monday-Friday 3:00 p.m.-11:00 p.m. with flexibility to work weekends and holidays as needed.

Annual Salary: $57119

Hampton-Newport News Community Services Board (H-NNCSB) is seeking a Service Access Coordinator to support service access patient flow care coordination and operational continuity during the evening at the B.J. Roberts Behavioral Health Center.

The B.J. Roberts Behavioral Health Center is an urgent care center for adults experiencing behavioral health or substance use crisis. The center supports the operations of the crisis programs including Emergency Services Marcus Alert 23-hr. Crisis Receiving Center CIT Assessment Center and the Regional Crisis Stabilization Center.

Minimum Qualifications
  • Masters degree in Human Services or a related field.
  • Two (2) years of clinical experience including therapeutic counseling crisis intervention and case management or care coordination functions with individuals with serious mental illness and/or co-occurring disorders.
  • Knowledge of Virginias behavioral health systems of care including crisis/emergency services is preferred.
About the Position

The Service Access Coordinator serves as the central point of coordination for service access and patient flow across multiple on-site crisis programs including Emergency Services Marcus Alert CIT Assessment Center the 23-Hour Crisis Receiving Center and the Crisis Stabilization Unit.

This position helps ensure individuals receive timely access to appropriate services by coordinating communication among programs assisting with referrals and transitions of care and addressing service-access issues in real time.

The Service Access Coordinator facilitates communication supports patient flow decisions within established protocols and promotes coordination among clinical teams programs community providers and healthcare partners.

Key Responsibilities
  • Coordinate referrals and connect individuals with appropriate services and providers based on their needs.
  • Conduct phone and face-to-face interviews with individuals families and care providers to assess needs and determine appropriate services.
  • Work with program managers administrators and the Director on complex situations requiring additional review or support.
  • Communicate with psychiatric and healthcare providers about available services transfers and step-down care for individuals served by the CRC and CSU.
  • Help individuals access community resources such as shelter food medical care transportation disability benefits and other available assistance.
  • Educate individuals and staff about available services and resources and assist with connecting individuals to appropriate supports.
