Senior Social Worker

Hall & Prior Health & Aged Care Group is a family-owned aged care provider operating 42 residential aged care homes with in-home and community care programs in WA and NSW. We are looking for a Senior Social Worker to support our team of Social Workers who work across our Transitional Care Program (TCP) Residential and Community as well as our Residential Time to Think (TTT) program. We are looking for someone who can join us part-time 3 to 4 days per week.

About the role

As a Senior Social Worker you will responsible for providing leadership clinical support and supervision of our Social Worker team. This role will involve undertaking comprehensive goal-oriented and timely psychosocial assessments to understand the needs and circumstances of our consumers. You will be provided with a small caseload of consumers and be required to support our team of social workers with problem solving complex cases and ensure discharges are timely and meeting the goals of our consumers. You will be supported and guided by our Allied Health Manager and Transition Care Program Manager with theopportunity to work closely with our Multi-disciplinary Team (MDT) including Occupational Therapists Physiotherapists Therapy Assistants as well as the residential and home care teams.

What youll bring

• Tertiary qualification in Social Work (Bachelor or Master of Social Work)
• Australia Association of Social Work (AASW) membership (or eligible to be an accredited member)
• Strong communication skills with an ability to mentor and support staff
• NDIS Worker Screening Check
• Influenza vaccination
• Full working rights in Australia
• Full Australian Drivers license
• Knowledge on SAT and ACAT processes
• Knowledge on the strengthened aged care standards and TCP guidelines
