Senior Resident Service Coordinator Beacon House Square - Baltimore MDMaster of Social Work (MSW) RequiredPosition Description:The Senior Resident Service Coordinator (Sr. RSC) develops and coordinates support services for residents. The Sr. RSC will ensure resident and community success through close work with property management clinical leadership and care coordination building and sustaining relationships with local and regional partners planning programs and other on-site opportunities for residents and working one-on-one with residents to assist them in achieving their goals and maintaining housing position requires a Master of Social Work (MSW) and includes lead responsibility for coordinating referrals and services with Project PLASE the on-site Permanent Supportive Housing (PSH) partner.A Sr. RSC is defined in one of the following three ways:Supervising other RSCs at the same property or at proximal propertiesResponsible for supporting proximal properties without an RSCComplexity/size of property requires someone with more experienceSenior Resident Service Coordinator Role:Provide strategic leadership for the property in creating and implementing programs partnerships and services that meet the needs of residents and fulfill any obligations based on property as the lead wellness and service coordination point of contact with Project PLASE the on-site Permanent Supportive Housing (PSH) partner ensuring clear referral pathways shared care plans and coordinated interventions for residents with complex behavioral health medical or housing stability guidance to the property management staff related to high-risk residents crisis situations and complex service include supervising RSCs both within the property and across the region and supporting properties without an in Sr. RSC leadership calls with other Sr. RSCs across the company and lead projects and tasks to further the goals of the meetings to plan for acquisition of new properties in the effective working relationships with Area Resident Service Manager (ARSM) RSC colleagues corporate staff and Property Management Role:Assess and coordinate resident referrals and resources by recommending solutions to resolve resident issues impacting their life and refer them to suitable community services resources and entitlement programs as relationships with residents to better understand their aspirations and goals and connect them with local resources and partnerships with local organizations to successfully implement community engagement initiatives including implementation of required elders or those at-risk and non-elderly residents with disabilities to maintain living independently through collaboration and coordination of programs with wellness nurse/health care agency to addresses common health concerns such as fall prevention nutrition grief and loss medication management dealing with incontinence healthy weight loss brain health/improving cognitive functions and general social-emotional coordination of resident support and resources on housing stabilization issues such as late rent payments housekeeping and other lease with property management team to develop program budgets that reflect needs of the resident population and that comply with program finance consistent outreach and communication with residents including completing new resident assessments newsletters flyers monthly calendars telephone calls and door Beacons data collection and analysis systems to document services provided to residents and outcomes and monitor the propertys Community Engagement Reasonable Accommodation requests for residents with with the property management team and attend weekly meetings as to resident crises including behavioral health emergencies in coordination with clinical providers emergency services and property Skills and Abilities:Experience working with older adults and issues relating to isolation health and wellness and other social determinants of working with individuals experiencing homelessness serious mental illness substance use and/or chronic health conditions including Permanent Supportive Housing working with chronically homeless veterans including familiarity with VA systems veteran benefits trauma-informed care and housing stabilization strategies for veteran working with youth and families and issues relating to workforce development youth enrichment education and asset to understand and respect values attitudes and beliefs that differ across cultures and to respond appropriately to these differences with residents in planning implementing and evaluating programs and leadership ability to manage and support a team to implement programs and services in a changing of building and maintaining positive relationships with a wide variety of stakeholders including but not limited to residents senior staff volunteers interns and community to collect track and understand data to assess programs and partnerships and inform with technology and an interest in learning how to integrate new technology-based tools and resources into in a timely manner with consistency and a high level of integrity and professionalism; provide excellent customer service to residents and leadership ability and interest in motivating and energizing communication organization problem-solving and writing skills.
