Senior Care & Community Outreach liaison

Hours - 8-5 Monday - Friday ON SITE

We are seeking a motivated relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales senior services outreach and provider relations experience.

In this position youll build referral partnerships educate the community and connect eligible seniors with life-changing care and support services.

What Youll Do

  • Develop and maintain referral relationships with hospitals physicians rehab centers senior centers and community organizations
  • Educate referral sources families and community partners on program benefits
  • Generate qualified referrals and support enrollment growth
  • Conduct presentations community events and outreach activities
  • Maintain territory plans and referral pipelines
  • Collaborate with enrollment clinical and operations teams to ensure smooth participant transitions

What Were Looking For

  • 2 years of experience in healthcare sales outreach business development or community engagement
  • Experience in senior care home care hospice PACE
  • Strong networking and relationship-building skills
  • Excellent communication and presentation skills
  • Self-motivated with ability to manage a territory independently
  • Comfortable in assisting seniors with the process in their homes
  • Valid drivers license and reliable transportation
