Job Description
Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field.
Requirements:
Coordination with the pharmacy on patient medications
Documentation of interventions in EHR
Telehealth appointment coordination
Clinical appointment coordination
Patient monitoring for personal hygiene room cleanliness and sobriety
Must be able to pass a drug and background check.
