Job Summary
Minimum Qualifications
Associate degree required; Bachelors preferred in Social Work Public Health Community Health Human Services or related field.
Two years of experience in community outreach case management patient navigation or social service delivery preferred.
Strong interpersonal communication and problem-solving skills.
Ability to work independently in community settings with limited supervision.
Preferred Qualifications
Experience working in rural or underserved communities.
Knowledge of social determinants of health and community-based resources.
Completion of a Community Health Worker training program (such as Partners in Care) preferred.
Ability to complete approved Community Health Worker training during the first year of employment preferred.
