As a Pediatric Home Care LPN with UnityPoint at Home youll work one-on-one with children and their families to develop the care plan that is right for them. Our pediatric nurses make it possible for patients to receive the care they need where theyre most comfortable in their homes

Ped Nurses provide skilled nursing care to pediatric-young adult patients with special health care needs for shifts of time in the patients home. Perform prescribed activities directed under the patients physicians care which may include; patient assessments feedings respiratory system management medication administration activities of daily living and therapy programs. Assist in care coordination with care team. May be responsible for transportation of patient.

• Graduate of State Board approved program for Licensed Practical Nurses and valid license Licensed Practical Nurse.
• CPR Certification.
• Strong interpersonal skills.
• Ability to work as a collaborative team Care Management (ICM) certification or successful completion of certification within one year of employment.
• Valid Mandatory Reporter course completion by state(s) requirement
• Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.
• Maintenance of an acceptable driving record (i.e. no OWI convictions in last 7 years or no more than 3 moving violations in last 3 years).
• An unacceptable driving record may result in suspension termination or transfer to another position not requiring the ability to drive.
