As a Home Health RN you will:

• Be a clinical leader. Apply your expertise directly in a patients most comfortable environment delivering high-quality personalized nursing care.
• Craft individualized care. Develop implement and dynamically adjust comprehensive care plans that promote patient independence and well-being.
• Collaborate and coordinate. Work seamlessly with a multidisciplinary team physicians therapists and support staff to ensure holistic care.
• Empower and educate. Serve as a vital resource for patients and their families providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload leading and nurturing LPNs LVNs and Home Health Aides to foster a strong cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking proven clinical expertise strong case management abilities and an unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid drivers license and reliable transportation.
