Job Summary

RN Post Acute Long-Term Care (LTC)

Location: Lewiston Maine
Duration: 13 Weeks

Shift: Days (7:00 AM 7:00 PM)

Responsibilities
  • Provide direct and indirect nursing care to residents in long-term care dementia care skilled nursing and rehabilitation settings.
  • Deliver compassionate resident-centered care to individuals with Alzheimers disease and other forms of dementia.
  • Perform comprehensive nursing assessments and develop individualized care plans.
  • Administer medications IV therapies and specialized treatments as prescribed.
  • Provide wound care management and monitor resident progress.
  • Support patients recovering from surgeries strokes fractures joint replacements and other acute conditions.
  • Collaborate with physicians therapists and interdisciplinary team members to ensure quality patient outcomes.
  • Educate residents and families regarding treatment plans and ongoing care needs.
  • Maintain accurate and timely clinical documentation.
  • Float between units as needed to support operational and patient care requirements.
Requirements
  • Active Maine RN License or Compact Multistate RN License.
  • Current BLS Certification.
  • Associate Degree in Nursing (ADN) or higher from an accredited nursing program.
  • Minimum 2 years of RN experience required.
  • Long-Term Care Skilled Nursing or Rehabilitation experience preferred.
  • Strong clinical assessment communication and organizational skills.
  • Ability to provide compassionate care to geriatric and rehabilitation populations.
Preferred Qualifications
  • Experience in dementia and memory care settings.
  • Skilled Nursing Facility (SNF) experience.
  • Experience with IV therapy and complex wound care.
  • Strong interdisciplinary collaboration and patient advocacy skills
