Job Summary
RN Post Acute Long-Term Care (LTC)
Location: Lewiston Maine
Duration: 13 Weeks
Shift: Days (7:00 AM 7:00 PM)
Responsibilities
- Provide direct and indirect nursing care to residents in long-term care dementia care skilled nursing and rehabilitation settings.
- Deliver compassionate resident-centered care to individuals with Alzheimers disease and other forms of dementia.
- Perform comprehensive nursing assessments and develop individualized care plans.
- Administer medications IV therapies and specialized treatments as prescribed.
- Provide wound care management and monitor resident progress.
- Support patients recovering from surgeries strokes fractures joint replacements and other acute conditions.
- Collaborate with physicians therapists and interdisciplinary team members to ensure quality patient outcomes.
- Educate residents and families regarding treatment plans and ongoing care needs.
- Maintain accurate and timely clinical documentation.
- Float between units as needed to support operational and patient care requirements.
Requirements
- Active Maine RN License or Compact Multistate RN License.
- Current BLS Certification.
- Associate Degree in Nursing (ADN) or higher from an accredited nursing program.
- Minimum 2 years of RN experience required.
- Long-Term Care Skilled Nursing or Rehabilitation experience preferred.
- Strong clinical assessment communication and organizational skills.
- Ability to provide compassionate care to geriatric and rehabilitation populations.
Preferred Qualifications
- Experience in dementia and memory care settings.
- Skilled Nursing Facility (SNF) experience.
- Experience with IV therapy and complex wound care.
- Strong interdisciplinary collaboration and patient advocacy skills
