Summary of Job Duties:
Registered Nurse will provide direct patient care as an RN. This professional nursing role will require the ability to coordinate care for patients triage patients and work collaboratively with clinic providers fellows and staff. Requires the ability to effectively work with patient referral sources facilitate appointments coordinate transitions of care and provide patient education on complex conditions. The RN will work with the individual patient their families and providers of care to develop and implement individual health plans.

Qualifications:

Minimum Qualifications:

  • Baccalaureate Degree BSN ADN Degree or Diploma RN. Major: Registered Nurse.

  • RN with a minimum of 1 year of RN experience.

  • Current BLS certification.

Preferred Qualifications:

  • ACLS certification or obtain certification within one year of hire date.

  • Knowledge or experience in PCMH desired.


Additional Information:

Responsibilities:

  • Performs comprehensive nursing assessments and identify patient health needs.

  • Develops and implement individualized care plans in collaboration with physicians nurse practitioners and other healthcare providers.

  • Coordinates patient care including referrals follow-up appointments diagnostic testing and continuity of care.

  • Educates patients and families on disease prevention health maintenance medications treatment plans and self-management strategies.

  • Manages prescription refill requests according to clinic policies and provider orders.

  • Monitors patient progress document assessments and interventions in the electronic medical record and communicate changes in patient status to providers.

  • Serves as a clinical resource for the Womens Center by developing expertise in womens health conditions and treatments.

  • Collaborates with an interdisciplinary team including physicians advanced practice providers social workers pharmacists and other healthcare professionals.

  • Supports quality improvement initiatives and implement evidence-based practices to improve patient outcomes.

  • Provides compassionate patient-centered care while maintaining patient privacy safety and regulatory compliance.

  • Assists with clinic procedures and ensure appropriate patient preparation and follow-up as needed.

  • May perform other duties as assigned
