Job Summary

Title: RN / MSW Care Coordination / Case Management Contract

Location: Everett Washington (Level II Trauma Center Multi Campus Facility)

Duration: 13 Weeks

Payrate:Starting at 45$/hour

Shift: 4x10 Day Shift (Start ASAP)

Position Overview

Navitas Healthcare LLC is seeking an experienced RN or MSW for a Case Management / Care Coordination role in an acute care hospital setting. The position involves discharge planning utilization review support and coordinating patient care across multiple hospital campuses.

Key Responsibilities
  • Perform discharge planning and care coordination for acute care patients
  • Collaborate with physicians nurses and interdisciplinary teams
  • Support case management activities across Med-Surg ED and long length-of-stay patients
  • Assess patient needs and develop safe discharge plans
  • Coordinate post-acute care services and community resources
  • Ensure timely documentation in EMR (EPIC)
  • Participate in care conferences and patient progress reviews
  • Assist with utilization and utilization review workflows
  • Work across multiple campuses as needed
  • Ensure smooth patient transitions and continuity of care
Required Qualifications
  • Active RN License (or MSW eligible for case management role)
  • Minimum 1 year of acute care experience required
  • Discharge planning / care coordination experience required
  • Strong preference for Med-Surg ED or LLOS case management experience
  • BLS Certification required (for RN roles)
  • Ability to work in fast-paced hospital environment
  • Strong communication and organizational skills
  • EPIC charting experience preferred
