We are seeking an experiencedRN Manager Case Managementto oversee the daily operations and clinical practice of the Case Management department at a respected acute care hospital in Santa Rosa California.

This leadership role is responsible for managing Case Managers and support staff while ensuring effective care coordination discharge planning utilization management and quality outcomes across the continuum of care. The manager will collaborate with physicians nurses social workers and interdisciplinary teams to ensure patients receive timely appropriate and coordinated care.

• Provide leadership and oversight for the Case Management department.

• Supervise mentor and develop Case Management staff.

• Ensure effective care coordination discharge planning and transition of care.

• Monitor department productivity quality metrics and regulatory compliance.

• Lead quality improvement initiatives and performance management activities.

• Manage departmental budgets staffing recruitment onboarding and retention.

• Collaborate with interdisciplinary teams to optimize patient outcomes and resource utilization.

• Ensure compliance with organizational policies accreditation standards and regulatory requirements.

• Bachelors degree in Nursing (BSN).

• Active California Registered Nurse (RN) license.

• Minimum 5 years of healthcare experience.

• Minimum 3 years of Case Management experience in an acute care/inpatient hospital setting.

• Minimum 1 year of healthcare leadership or management experience.

• Masters degree in Nursing Healthcare Administration or a related field.

• Certified Case Manager (CCM) or Accredited Case Manager (ACM).
