Schedule: Mid Shift 5x8-Hour Shifts
Shift Timing: 2:30 PM 10:45 PM
Weekly Hours: 40 Hours
Job Summary:
Navitas Healthcare LLC isseeking an experienced RN Long Term Care (LTC RN) to provide compassionate and individualized nursing care within a rehabilitation and nursing facility setting. The RN will assess plan implement and evaluate patient care while supporting residents with memory care rehabilitation skilled nursing and chronic health conditions. The ideal candidate will possess strong clinical judgment effective communication skills and the ability to work efficiently in a long-term care environment.
Key Responsibilities:
Assess plan implement and evaluate individualized resident care plans
Provide nursing care for long-term care and rehabilitation residents
Care for residents with Alzheimers and dementia-related conditions
Monitor patient conditions and communicate changes to healthcare providers
Administer treatments and perform nursing interventions as ordered
Document patient care accurately within facility standards
Collaborate with interdisciplinary healthcare teams to coordinate care
Provide patient and family education regarding care plans and treatments
Maintain infection control and patient safety standards
Support resident comfort dignity and quality of life initiatives
Required Skills & Experience:
Minimum 1 year LTC experience required
Candidates with recent rehab med-surg or hospital experience will be considered
Strong communication and patient care skills
Ability to work effectively in a long-term care setting
Experience caring for dementia and Alzheimers patients preferred
First-time travelers accepted
Education Requirements:
Graduate from an accredited nursing program required
ASN or Diploma in Nursing required
BSN preferred
Certifications & Licensure:
Active New York RN License required
BLS Certification required
Proof of NY License required at time of submission
No license hits allowed
