Overview:

Sentara MJ is hiring an RN Integrated Care Manager to support Primary Care Practices in the Charlottesville area.

Job Summary

  • Monday through Friday 40 hours per week
  • Standard work hours are 8:00 AM to 4:30 PM
  • Remote position; candidates must reside in the Charlottesville area
  • Travel to designated locations is required 14 days per month

The RN Integrated Care Manager is responsible and accountable for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum. Promotes effective utilization and monitoring of health services collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs. Functions in one of the following practice settings: Acute Care Service Lines Ambulatory/Community-based Home Health and Long-Term Care.

Experience:

  • 3 years of nursing experienceRequired
  • 3 years of Case Management experience preferred

Education:

  • RN Diploma Required
  • ASN BSN or MSNpreferred

Certification:

  • VA Registered Nurse License or compactRequired
  • BLS required within 90 days of hire and maintained thereafter (if in clinical setting)
  • For Integrated Care Management departments specialty certification required within one year of eligibility (ACM CCM CCCTM CMAC or CGMT-BC).
  • For other service lines certification based on specialty area required within one year of eligibility.
  • For those in Behavioral Health - Certification in de-escalation training within 15 days of hire and annually.
