Position Summary:
The Home Health Registered Nurse (RN) is responsible for providing skilled nursing care coordinating patient services and ensuring continuous assessment of patients physical medical psychosocial emotional and spiritual needs. This role plays a key part in developing and implementing individualized plans of care for patients in a home setting.
Patient Care & Case Management:
Position Summary:
The Home Health Registered Nurse (RN) is responsible for providing skilled nursing care coordinating patient services and ensuring continuous assessment of patients physical medical psychosocial emotional and spiritual needs. This role plays a key part in developing and implementing individualized plans of care for patients in a home setting.
Patient Care & Case Management:
- Perform admissions skilled nursing visits and follow-up care for home health patients
- Provide ongoing case management and coordinate patient care
- Conduct comprehensive assessments and develop individualized care plans
- Collaborate with physicians to develop and implement treatment plans
- Revise care plans based on patient progress and ongoing assessments
- Coordinate care with interdisciplinary team members (PT OT HHAs and other providers)
- Deliver hands-on nursing care and patient education in accordance with physician orders
- Monitor patient condition and respond to changes appropriately
- Ensure continuity and quality of care
- Communicate patient status progress and concerns to physicians and Clinical Manager
- Maintain accurate timely and compliant documentation
- Supervise and ensure proper documentation by home health aides and support staff
- Associate or Bachelors degree in Nursing
- Active and unrestricted Florida RN license
- Current CPR certification
- Minimum 1 year of nursing experience (preferred)
- Strong clinical background (Med-Surg ICU ER or acute care experience highly valued)
- Home health experience preferred but not required
