About the PositionThe primary goal of the Home Health Registered Nurse is to help patients return to their highest practicable level of independence and well-being within the community through effective self-management of their health conditions.
When independence is not achievable the nurse focuses on preparing the patient family and caregivers for a seamless continuum of care within the home or other community-based healthcare systems.
Qualifications-
Education: Graduate of an accredited approved school of professional nursing.

Licensure: Active Registered Nurse (RN) license in the State of Colorado in good standing.

Experience: Minimum of one (1) year of experience as a Registered Nurse. Home health experience is highly preferred.

Skills: Familiarity with OASIS documentation is preferred. Must be able to communicate effectively in the patients language and document all patient-related activities accurately.

Transportation: Valid drivers license and reliable transportation.
